The Coordinated Veteran's Care ProgramThe Coordinated Veteran's Care Program (CVC Program) is a new Department of Veterans Affairs (DVA) program to better manage and coordinate primary and community care for Gold Card holders who are most at risk of being admitted or readmitted to hospital. Gold Card holders include veterans, war widow, war widowers and dependants. The CVC Program aims to provide additional support for Gold Card holders with one or more chronic disease or conditions and who have been identified a having complex care needs. The CVC Program uses a team approach with the core team being made up of the participant, the participant's carer (if applicable), the GP and the nurse coordinator. The team uses care planning, coordination and review as tools to provide better chronic disease management and encourage a self management approach from the CVC participant. GPs are able to enroll participants in the program by completing an eligibility assessment, and obtaining informed consent from the eligible participant. GPs are able to claim an initial incentive payment and ongoing quarterly payments for each Gold Card holder that is enrolled in the program that they are providing ongoing care to.
