Type 2 Diabetes - Risk Assessment and Management

Assess a patients risk of developing diabetes using the AUSDRISK questionnaire (pdf version or MD template)

 

MBS Items for Diabetes Risk Evaluation

A medical practitioner may select MBS item 701 (brief), 703 (standard), 705 (long), or 707 (prolonged) to undertake a type 2 diabetes risk evaluation depending on the length of the consultation as determined by the complexity of the patient's presentation. This can either be as a:

Health assessment for people aged 40-49 years with a high risk of developing type 2 diabetes or

Health assessment for people aged 45-49 years who are at risk of developing chronic disease.

For indigenous patients aged 15-54 years, a medical practitioner may use MBS item 715: Health Assessment for Aboriginal and Torres Strait Islander people, after scoring 12 or greater on the AUSDRISK tool.

We wish to advise that funding for the Lifestyle Modification Program (LMP) has now been discontinued by the Commonwealth nationally. As a result ISML can no longer accept referrals to our Program “Reset your Life  however, Group Programs currently underway will be completed by early 2012.

Alternative referral sources for lifestyle support are listed below.

Initiate interventions to reduce risk or manage type 2 diabetes

The following are options to provide patients with who need to make lifestyle changes to reduce their diabetes risk:

  • Refer patients to the free telephone-based health coaching service.                 Ph: 1300 806 258 www.gethealthynsw.com.au
  • Refer patients to allied health professionals to assess specific lifestyle factors and provide advice and encouragement to improve health outcomes. A list of local allied health professionals can be found in our Diabetes Information Booklet and Service Directory
  • Patients with existing type 2 diabetes are entitled to 8 Medicare group education sessions run by a diabetes educator, dietitian or exercise physiologist. Click here for the referral form.

 

Active Learning Module (ALM)

Prevention of type 2 diabetes: Identification and Management of High Risk Patients

The prevention of type 2 diabetes online Active Learning Module has been developed by the Australian General Practice Network (AGPN) and is funded by the Australian Government. This is a 6 hour module and is approved for Category 1 and Category 2 RACGP points and is also approved with ACRRM.
