You will need to:

1. Complete a Medicare card application form available from:
• a Medicare Office, or
• download a form
2. You will need:
• the number of the Medicare care you are currently on (most probably your parent's card)
3. You need to prove who you are and that you are over 15 years of age. Take at least 2 of the following forms of ID to the medicare office:
• birth certificate
• passport
• bank book (if your address is on it)

With this information they will be able to take your name off your parent's care and give you your own number on your own card.

Once you have lodged your application for a Medicare card, your own card with your own number will be sent to you:

• You can make arrangements with medicare to pick up your card at the office or an address that you specify.
